矮小症作为儿科内分泌的常见病,是指在相似环境下身高较同年龄、同性别、同种族健康儿童身高均值低2个标准差(-2SD)或处于第3百分位以下[1]。生长激素/胰岛素样生长因子-1轴(Growth hormone/ insulin-like growth factor-1,GH/IGF-1)是调控生长发育的主要因素。生长激素(growth hormone,GH)作为垂体前叶激素,是脂代谢的关键调节因子[2]。而肝脏是GH的主要靶点,研究表明肝脏GH受体或其下游信号通路功能障碍可升高细胞内脂质积累和促进代谢相关性脂肪肝病(metabolic associated fatty liver disease, MAFLD)发展[3-4]。
MAFLD作为2020年国际共识提出的一个新概念,是全球肝病的主要病因,影响全球近25%的人口[5]。MAFLD与高血压、胰岛素抵抗、糖尿病、血脂异常、肥胖和高尿酸血症等多种因素有关[6-8]。血清丙氨酸转氨酶(alanine aminotransferase, ALT)水平升高是反映肝细胞损伤、筛选MAFLD及反映肝脏组织学进展的一种有价值的指标[9-12]。
胰岛素样生长因子-1(insulin-like growth factor-1, IGF-1)的表达受尿酸(uric acid, UA)水平影响[13]。UA是嘌呤代谢的天然终产物,其偏差可反映代谢紊乱。一些研究表明血清UA水平与MAFLD存在相关性,主要多发于肥胖、胰岛素抵抗和代谢综合征的患者中[8, 14]。然而,关于UA与ALT间相关性的研究在矮小症儿童和青少年中较少。本研究旨在探讨儿童和青少年矮小症患者UA水平与ALT水平的相关性。
1 资料和方法 1.1 一般资料选取2013年03月01日至2019年02月28日于山东省生长发育疾病随访研究队列(ChiCTR1900026510)中诊断为矮小症的儿童和青少年患者685例为研究对象。纳入标准:1)符合矮小症诊断标准;2)出生时体重和长度在同胎龄儿正常范围;3)至少完成L-左旋多巴和胰岛素两次GH激发试验中的一项。排除合并以下疾病者:1)甲状腺功能障碍;2)颅脑肿瘤; 3)特纳综合征; 4)先天性心脏病;5)慢性肝脏或肾脏疾病;6)营养不良;7)先天性代谢性疾病;8)染色体异常等疾病。在排除数据缺失后,共纳入患者685例,其中男性496例,女性189例,平均年龄(10.1±3.55)岁。济宁医学院附属医院伦理委员会批准该研究,患儿家属均签署知情同意书。
1.2 生物化学指标检测 1.2.1 基本参数所有参与者均由经过培训的专业护士用相同的身高测量仪器测量身高,用相同的称重秤测量体重。体重指数(body mass index, BMI)计算为体重(kg)/身高(m2)。根据中国同年龄同性别儿童身高中位数分别计算身高标准差积分(height standard deviation score, HT SDS)及体重指数标准差积分(body mass index standard deviation score, BMI SDS)[15-16]。以上指标均在患者脱鞋、空腹状态下测得。根据Tanner分期,通过体格检查评估青春期阶段。以下标准用于定义青春期前:对于男孩,睾丸体积小于4ml,没有阴毛;对于女孩,没有乳房发育,没有阴毛[17-19]。
1.2.2 生化指标所有患儿均为清晨抽取空腹静脉血,用酶法(上海罗氏公司)测定肝肾功能[ALT、天冬氨酸转氨酶(aspartate aminotransferase, AST)和UA]及血脂[总胆固醇(total cholesterol, TC)、甘油三酯(triglyceride, TG)、高密度脂蛋白(high density lipoprotein, HDL)、低密度脂蛋白(low density lipoprotein, LDL)]和空腹血糖(fasting blood glucose, FBG)等生化指标。
1.2.3 激素测定患儿均进行两项生长激素激发试验,即左旋多巴生长激素激发试验和胰岛素低血糖生长激素激发试验,并选择其在0、30、60、90、120min测得最大值作为生长激素峰值(GH peak)。采用化学发光法测量生长激素。采用发光免疫量度法测量IGF-1,并计算IGF-1标准差积分(insulin-like growth factor standard deviation score, IGF-1 SDS)[20]。
1.3 统计学方法所有统计分析均使用易侕统计软件。计量资料以x±s表示,连续变量P值用Kruskal Wallis秩和检验得出。采用单因素回归分析检验多个因素与ALT之间的关系。使用广义相加混合模型实现平滑曲线拟合,分析UA水平与ALT的相关关系。通过对数似然比检验来确定二者是否存在曲线关系,若存在曲线关系,使用递归算法实现阈值效应分析并计算拐点,同时在拐点两侧构建双分段回归模型。P < 0.05为差异有统计学意义,并计算95%可信区间。
2 结果 2.1 研究对象的基线资料特征685例矮小症患者中,男孩496例,女孩189例,平均年龄(10.1±3.55)岁,平均身高为(125.0±18.28)cm,平均UA水平(4.44±1.23)mg/dl,ALT(15.84±9.14)U/L。将研究人群按照UA水平值四分位数分成4组,Quartile 1组:血UA水平(0.51~3.61)mg/dl,170例;Quartile 2组:血尿酸水平(3.62~4.30)mg/dl,169例;Quartile 3组:血UA水平(4.31~5.07)mg/dl,173例;Quartile 4组:血UA水平(5.08~10.03)mg/dl,173例。4组间UA水平存在显著性差异(P < 0.001)。基线年龄、身高、体重、ALT、HDL在4组中比较,差异有统计学意义(P < 0.05),且随着UA水平越高,HDL越低(P < 0.05)。见表 1。
表 2为患者一般资料及生化指标与ALT的单因素分析,结果显示UA、年龄、IGF-1、BMI SDS与ALT水平呈正相关(P < 0.05)。GH peak与ALT水平呈负相关(P < 0.05),而IGF-1 SDS、Tanner分期、TC、TG、LDL、HDL与其无相关性(P>0.05)。
图 1通过平滑曲线拟合模块拟合矮小症患者UA与ALT的相关关系,结果显示在调整年龄、性别、IGF-1、IGF-1 SDS、BMI SDS、Tanner分期、TG、TC、HDL、LDL、FBG、GH peak等混杂因素后,UA与ALT呈曲线关系,并存在一个拐点。在拐点前,随着UA增加, ALT呈下降趋势; 在拐点后, 随着UA增加, ALT呈上升趋势。
表 3校正了年龄、性别、IGF-1、IGF-1 SDS、BMI SDS、Tanner分期、TG、TC、HDL、LDL、FBG、GH peak等混杂因素以后,通过阈值效应分析发现,UA的拐点为6.47mg/dl。当UA < 6.47mg/dl时,UA每增加1mg/dl, ALT降低0.04U/L(95%CI:-0.69~0.76;P=0.924);当UA≥6.47mg/dl时,UA每增加1mg/dl,ALT增加3.62U/L(95%CI:0.85~6.40;P=0.011)。
UA水平与MAFLD的发生密切相关[8, 14]。Yang等[21]对7569名平均年龄为59.8岁的中国人群进行横断面研究,结果显示男性最高UA水平组的MAFLD患病率与最低水平组相比高2.74倍,女性高4.60倍,且UA水平与轻度和重度脂肪变性的患病率显著相关。说明无论是在男性还是女性中,UA和MAFLD患病率均呈正相关。在超重或肥胖的儿童和青少年人群中,亦有同样发现。一项对2565名超重或肥胖的年轻人(5~18岁)的横断面研究,发现高水平UA与超重或肥胖患者的MAFLD发生独立相关,UA可作为识别MAFLD危险的标志[22]。队列研究方面,一项对我国南方6890名患者的随访研究,结果显示随着UA水平升高,MAFLD的发病率逐渐增加[23]。
本项研究对矮小症儿童和青少年人群进行横断面分析,分组研究显示,UA水平较高组ALT水平较高,这与Yang等[21]研究结果一致。进一步分析发现UA和ALT之间存在曲线关系,UA拐点为6.47mg/dl。只有UA水平超过这一点时,UA和ALT之间呈显著的正相关。目前临床研究同样证实了UA水平升高是MAFLD发生与进展的危险因素:在基于糖尿病和肥胖患者的两项研究中均发现UA预测MAFLD的截止值为>6mg/dl[24-25]。而本文研究发现矮小症患者的UA水平高于拐点时,UA与ALT呈正相关。而与之不同的是,我们得出UA的截止值为6.47mg/dl。这可能与研究人群、调整混杂不同有关。我们的研究是基于矮小症儿童和青少年人人群,既往较少的文献研究这一群体,因此,我们结论背后潜在的机制仍需进一步的研究。理论上,我们假设UA刺激脂肪积累、肝脏脂肪变性和肝脏炎症,这些因素可能在MAFLD的发病过程中起着至关重要的作用。
本研究存在一定的局限性。首先,由于横断面研究设计,我们无法确定因果关系。其次,影响UA水平的因素很多,如饮食和营养状况。最后,我们将使用饮食和运动问卷来更好地评估身材矮小的儿童和青少年的饮食及营养状况。
总之,UA血症与MAFLD均与人体代谢密切相关,而我们观察到矮小症儿童和青少年患者中UA血症比例较高[26],UA与ALT之间存在曲线关系。因此,在矮小症患者的诊疗工作中必须重视UA水平检测。
