      目的:探讨Omega-3 多不饱和脂肪酸对精神分裂症患者认知功能的改善作用。方法:纳入45例服用非典型抗精神病药物治疗的精神分裂症患者,试验组患者(30例)在原有非典型抗精神病药物治疗基础上增加口服4g/d Omega-3 多不饱和脂肪酸鱼油胶囊治疗,对照组患者(15例)原有治疗药物不变,共治疗12周。在治疗前及治疗12周后对两组患者采用MATRICS公认认知成套测验(MATRICS Consensus Cognitive Battery,MCCB)进行认知功能测评;用气象色谱仪法分析红细胞膜中二十碳五烯酸(eicosapentaenoic acid,EPA)及二十二碳六烯酸(docosahexenoic acid,DHA)含量。结果:治疗前两组患者各项评分无显著性差异(P >0.05),治疗12周后,试验组患者符号编码、迷宫、范畴流畅、Stroop色词测验-色测验评分明显高于对照组(P<0.05)。干预12周后试验组患者连线测验耗时明显减少,符号编码、迷宫、范畴流畅、词测验评分明显提高,差异有统计学意义(P <0.05);对照组患者治疗前后各项评分均无显著性差异(P >0.05);试验组患者治疗前后迷宫、范畴流畅测验评分差值较对照组明显提高,差异有统计学意义(P <0.05)。治疗前两组患者红细胞膜EPA和DHA含量比较无显著性差异(P >0.05),治疗12周后,试验组患者红细胞膜中EPA与DHA含量明显高于对照组(P <0.05),治疗12周后试验组患者红细胞膜EPA及DHA含量较治疗前显著升高(P <0.05),对照组患者红细胞膜EPA及DHA含量较治疗前无明显差异(P >0.05)。试验组患者治疗前后红细胞膜EPA及DHA含量差值较对照组显著升高(P <0.05)。结论:Omega-3多不饱和脂肪酸鱼油胶囊对精神分裂症患者在信息处理速度、推理和问题解决能力领域的认知功能具有显著的改善作用,进一步的研究应进行更大的样本量来明确结论。

      Objective: To investigate the effect of omega-3 polyunsaturated fatty acids on cognitive function in patients with schizophrenia. Methods: a total of 45 schizophrenics treated with atypical antipsychotics were included. The experimental group (30 cases) was treated with oral 4G /d omega-3 polyunsaturated fatty acid fish oil capsule on the basis of the original atypical antipsychotics, and the control group (15 cases) was treated with the same original drugs for 12 weeks. MATRICS Consensus Cognitive Battery (MCCB) was used to evaluate the Cognitive function of patients in both groups before treatment and 12 weeks after treatment. The contents of Eicosapentaenoic acid (EPA) and docosahexenoic acid (DHA) in erythrocyte membrane were analyzed by meteorological chromatograph. Results: Before treatment, there was no significant difference in the scores of the two groups (P >0.05). After 12 weeks of treatment, the symbol coding, maze, category fluency and Stroop word test-color test score of the experimental group were significantly higher than those of the control group (P<0.05). After 12 weeks of intervention, the time of connection test in experimental group was significantly reduced, and the symbol coding, maze, category fluency and word test scores were significantly improved, with statistical significance (P<0.05). There were no significant differences in scores of control group before and after treatment (P>0.05). The score difference of maze and categorical fluency test in experimental group was significantly higher than that in control group before and after treatment, with statistical significance (P<0.05). Before treatment, there was no significant difference in the contents of EPA and DHA in erythrocyte membrane between the two groups (P >0.05). After 12 weeks of treatment, the contents of EPA and DHA in erythrocyte membrane in experimental group were significantly higher than those in control group (P<0.05). After 12 weeks of treatment, the contents of EPA and DHA in erythrocyte membrane of experimental group were significantly increased compared with those before treatment (P <0.05), while the contents of EPA and DHA in erythrocyte membrane of control group were not significantly different compared with those before treatment (P>0.05). The content difference of EPA and DHA in erythrocyte membrane in experimental group was significantly higher than that in control group before and after treatment (P <0.05). Conclusions: Omega-3 polyunsaturated fatty acid fish oil capsules can significantly improve cognitive function in the areas of information processing speed, reasoning and problem solving ability in schizophrenia patients, and further studies should be conducted with a larger sample size to confirm the conclusions.
