| 目的 探讨单一体位完全后腹腔镜肾盂输尿管癌根治术治疗上尿路上皮癌的疗效。方法 收集2013年7月-2016年12月在济宁医学院附属医院入院治疗的上尿路上皮癌患者62例。随机分为观察组30例和对照组32例，观察组行单一体位完全后腹腔镜肾盂输尿管癌根治术，对照组行后腹腔镜联合下腹部小切口肾输尿管全长切除术。统计两种手术方式的手术时间、手术出血量、术后引流量、引流管拔除时间、尿管拔除时间、住院时间，并进行长期随访观察，比较两组患者手术有关指标及术后的复发情况。结果 观察组30例手术和对照组32例手术均顺利完成，观察组较对照组的手术时间短、手术出血量少、术后引流量少、术后拔除引流管时间短、术后拔除尿管时间短、住院时间短，差异具有统计学意义（P<0.05）。所有患者均随访1~41个月，随访患者暂无复发表现，差异无统计学意义（P>0.05）。结论 单一体位完全后腹腔镜肾盂输尿管癌根治术临床疗效满意，患者创伤小，手术效果好，是一种安全、有效的治疗方法。 |

| Objective To evaluate the efficacy of complete retroperitoneal laparoscopic radical nephrectomy for upper urinary tract neoplasms.Methods From July 2013 to December 2016,62 patients diagnosed with upper urinary tract neoplasms,were collected in Jining Medical University Affiliated Hospital.The patients were randomly divided into observation group (30 cases) and control group (32 cases).The patients in the observation group underwent complete retroperitoneal laparoscopic nephroureterectomy with single posture and the control group received radical retroperitoneal laparoscopic nephroureterectomy and partial resection of the bladder.The time of operation,the amount of blood loss,Induced flow,the time of extraction of drainage tube and the time of removal of the catheter were statistically analyzed.There was no statistically significant difference between the two surgical methods.Results In the observation group,30 cases of operation and 32 cases of the control group were successfully operated.Compared with the control group,the operation time was shorter,the bleeding volume was less,the postoperative drainage was less,the drainage time was short after the operation,the time of removal of the catheter was short,the hospitalization time was short,the difference was statistically significant (P<0.05).All patients were followed up for 1 to 41 months,follow-up of patients with no recurrence,the difference was not statistically significant (P>0.05).Conclusion Complete retroperitoneal laparoscopic nephroureterectomy with single posture for upper urinary tract neoplasms is safe and effective.It is a safe and effective treatment for the patients with small trauma and good operative effect. |
