| 目的 探讨富亮氨酸胶质瘤失活1蛋白(leucine-rich gliomainactivated-1,LGI1)抗体阳性边缘叶脑炎临床特点、诊断、治疗及预后。方法 对近期收治的1例LGI1抗体脑炎患者的临床症状、影像学检查结果及诊治过程进行总结性回顾,进而对该患者的整个诊治过程进行分析与探讨。结果 男性患者临床表现为精神行为异常及癫痫发作,抗癫痫药物抵抗,颅脑MRI提示双侧颞叶内侧异常信号,血和脑脊液检测显示LGI1抗体阳性。经激素冲击治疗,效果良好,后联合静注丙种球蛋白,达到临床痊愈。结论 LGI1抗体脑炎是一种具有独特临床表现的自身免疫性疾病,通过血清学检测可明确诊断,对患者早期正确诊断并及时开展免疫干预治疗,能够达到良好的预后。 |

| Objective To investigate the clinical characteristics,diagnosis,treatment and prognosis of leucine-rich glioma inactivated-1 (LGI1)-antibody associated limbic encephalitis (LE).Methods The clinical symptoms,imaging findings,diagnosis and treatment of one patient with LGI1 were retrospectively reviewed.to analysis and discuss the whole process of diagnosis and treatment.Results Male patients showed mental behavior,seizures and antiepileptic drug resistance.Brain MRI showed abnormal signal in bilateral temporal lobe.Blood and cerebrospinal fluid examination showed LGI1 antibody was positive.The effect is good for the patient with the hormone shock therapy,after joint intravenous immune globulin,achieve clinical recovery.Conclusion LGI1 antibody encephalitis is a autoimmune diseases with unique clinical manifestations.Serological testing can confirm the diagnosis of the disease.Patients with early diagnosis and timely immunization intervention could achieve a good prognosis and effectively cure. |
