      目的 与超声心动图(ECG)检查对照,评估双源CT(DSCT)心电门控管电流调节模式心脏扫描对冠心病患者左心室功能评估的准确性。方法 50例冠心病患者进行心电门控管电流调节模式下DSCT检查及经胸二维ECG检查(双平面Simpson法),将ECG对心功能参数的测量结果作为对照,评估DSCT测量值的准确性。结果 DSCT所获心脏舒张末期时相范围为90%~100% R-R间期,收缩末期时相范围为30%~40% R-R间期。DSCT心内、外膜轮廓手动校正前左心室舒张末期容积(EDV)、收缩末期容积(ESV)、每搏输出量(SV)测量值均大于校正后(P<0.001),但校正前后左室射血分数(LVEF)测量值差异无统计学意义(P>0.05)。DSCT与ECG对EDV、ESV、SV及LVEF的测量值差异均无统计学意义(P>0.05),且两种检查方法所得的各项测量值具有高度相关性(P<0.001),Bland-Altman分析示两种检查方法的测量结果一致性较好。结论 双源CT回顾性心电门控结合心电门控管电流调节技术能满足左心功能分析的需要,对左心室形态学参数及其功能的评估准确、可靠。
英文摘要:
      Objective To assess the reliability of left ventricular(LV) function analysis using dual source CT(DSCT) with ECG-gated tube current modulation in comparison with echocardiography(ECG) imaging.Methods 50 patients with coronary artery disease were involved in this study,retrospectively ECG-gated DSCT with tube current modulation as well as ECG imaging of the heart were performed.Pearson's correlation coefficient and Bland-Altman analysis were used to determine agreement for parameters of LV function.Results On DSCT end-diastolic and end-systolic images were identified at 90% to 100% and 30% to 40% of the R-R interval.Manual correction of LV circumscription minimized the value of End-diastolic volume(EDV),end-systolic volume(ESV) and stroke volume(SV),but made no significant difference for left ventricular ejection fraction(LVEF).The DSCT measurements of EDV,ESV,SV and LVEF had no significant difference from those of ECG(P>0.05).All CT measurements of LV function correlated well with those from ECG:for EDV(P<0.001).Bland-Altman plots confirmed these findings showing no systematic deviations between the different imaging techniques.Conclusion Retrospectively ECG-gated DSCT with auto tube current modulation doesn't hamper the correctly quantitative assessment of LV function,which can provide highly reliable measurements.
