| 目的 探讨风湿性心脏瓣膜病合并左房血栓的诊断及处理的经验。方法 回顾分析2002年1月至2009年12月我院心脏外科手术治疗风湿性心脏瓣膜病合并左房血栓97例。所有患者均行左房血栓清除术,同期行二尖瓣置换术(MVR)73例,二尖瓣及主动脉瓣置换术(DVR)24例,三尖瓣成形术(TVP)81例,左房折叠13例。结果 术后生存96例,死亡1例,脑、四肢并发症2例。随访2月~6 a,心功能均有不同程度改善,无栓塞发生。结论 对风心病合并左房血栓的患者,准确诊断、术中操作轻柔并彻底清除血栓、术后合理抗凝治疗,可避免在围手术期及远期出现脑、四肢动脉栓塞,降低病死率。 |

| Objective To investigate the experience on surgical management and diagnose of left atrial thrombus in patients with rheumatic heart valve disease.Methods 97 patients with rheumatic heart valvular disease combined with left atrial thrombus were treated surgically from January 2002 to November 2009.The elimination of left atrial thrombus was performed in all patients.Results The survivors were followed up for 2 months to 6 years.Their heart function improved and arterial embolization was not found.Conclusion Careful perioperative management, correcting diagnosis, complete elimination of thrombus and reasonable anticoagulation can avoid the arterial embolization and decrease the mortality. |
