| 颈肩腰腿痛是临床常见病,椎间盘退变(intervertebral disc degeneration,IDD)后产生的髓核突出压迫和刺激脊神经根导致的神经病理性疼痛是颈肩腰腿痛的常见原因。随着生活节奏加快,颈肩腰腿痛日渐年轻化,病情严重者会丧失劳动能力甚至致残,给个人、家庭和社会带来了巨大的医疗和经济负担。如何延缓和治疗IDD已成为医疗领域的热点和难点。富血小板血浆(platelet-rich plasma,PRP)治疗IDD是一种新型无创的非手术疗法,不仅可以改善退变椎间盘的微环境,而且对髓核细胞有着促增殖、促分化、促进基质合成和抗凋亡的作用,且PRP与间充质干细胞的联合应用或许会带来更好的疗效。然而由于PRP的制备标准不统一,给规范化治疗带来了严峻挑战。本文对PRP在治疗IDD上的最新研究成果进行了梳理和综述。 |

| Neck,shoulder,waist and leg pain is a common clinical disease,and neuropathic pain caused by the compression of the nucleus pulposus and stimulation of the spinal nerve root after intervertebral disc degeneration(IDD) is a common cause of neck,shoulder,waist and leg pain.With the accelerated pace of life,neck,shoulder,waist and leg pain is becoming increasingly younger,and in severe cases,it can lead to loss of labor ability or even disability,posing a huge medical and economic burden to individuals,families,and society.How to delay and treat IDD has become a hot and difficult topic of global research.Platelet rich plasma (PRP) is a new non-invasive non-surgical treatment for IDD.It can not only improve the microenvironment of degenerative intervertebral discs,but also promote the proliferation,differentiation,matrix synthesis and anti-apoptosis of nucleus pulposus cells.Simultaneously the combined application of PRP and mesenchymal stem cells may bring better therapeutic effects.However,due to the inconsistent preparation standards of PRP,it poses a serious challenge to the research of standardized treatment.This article summarizes the latest research results of PRP in the treatment of intervertebral disc degeneration,with the aim of better clinical application of PRP. |
