| 目的 分析严重增生性糖尿病视网膜病变行玻璃体切割手术后的疗效。方法 连续选取2008年1月至于至2012年12月在我院行玻璃体切割手术的严重增生性糖尿病视网膜病变患者48例48眼进行回顾性分析。行常规20G玻璃体切割手术,全视网膜光凝,硅油填充31例,长效气体填充11例,12例患者因合并白内障同时行晶状体超声乳化术,一期植入人工晶状体(IOL)3例。记录患者最终视力情况及视网膜状况。结果 术后视力提高29眼(60.4%),视力不变6眼(12.5%),下降13眼(27.1%)。术后出血致玻璃体积血5眼,视网膜脱离2例,取油后视网膜脱离1例,新生血管性青光眼4眼,角膜失代偿2眼,视网膜分支动脉阻塞1眼。结论 严重增生性糖尿病视网膜病变术后并发症多,预后较差,但仍有一定手术价值。 |

| Objective To analyse the vitrectomy surgery effect of severe proliferative diabetic retinopathy. Methods The vitrectomy surgery effect of 48 cases with severe proliferative diabetic retinopathy was analysed from January 2008 to December 2012. 20-gauge vitrectomy was performed in these 48 patients with pan-retinal photocoagulation and silicone oil filling in 31 cases, or perfluoropropane gases filling in 11 cases. Phacoemulsification was performed in 12 cases with cataract, and intraocular lens was implanted simultaneously in 3 cases. The final visual acuity and retinal condition were recorded. Results Visual acuity was improved in 29 cases (60.4%), maintained in 6 cases (12.5%), and decreased in 13 cases (27.1%). Postoperative vitrous hemorrhage occurred in 5 cases, retinal detachment occurred in 2 cases, and occurred in 1 case after silicone oil removed, neovascular glaucoma occurred in 4 cases, corneal decompensation occurred in 2 cases, and branch retinal artery occlusion occurred in 1 case.Conclusion Many postoperative complications of the vitrectomy surgery of severe proliferative diabetic retinopathy occur. Although the prognosis of the vitrectomy surgery of severe proliferative diabetic retinopathy is not satisfied in all cases, visual acuity is improved or maintained in most cases. So the surgery is still worth performing. |
